Heart of Hospice, part of the Optum family, is seeking a Social Worker to provide in-home medical social services under physician direction within a hospice program. The role requires a Texas Social Worker license, a Master’s in Social Work, and a driver’s license with reliable transportation; travel within a 60‑mile radius is expected.
Full-time position with standard hours (Mon–Fri 8:00 AM–4:30 PM) and occasional evenings/weekends.
