- Completes thorough patient assessment addressing all pathophysiologic and psychosocial needs.
- Identifies key nursing diagnoses from assessment data; prioritizes them with consideration of cultural, religious, and spiritual needs.
- Establishes a data‑driven individualized plan of care.
- Implements individualized plan of care and discharge plan; coordinates with other disciplines.
- Utilizes critical thinking to adapt plan of care in response to changes in patient needs.
- Evaluates outcomes of the plan of care and modifies as needed.
- Recognizes and intervenes in situations related to patient safety.
- Organizes and prioritizes work assignments based on patient acuity.
- Demonstrates effective nursing judgments and documents progress legibly and timely.
- Documents nursing interventions and patient responses in accordance with hospital policy.
- Advocates for patient well‑being (religious/cultural preferences, privacy, etc.).
- Communicates patient data clearly, concisely, and timely in written and verbal form.
- Identifies and evaluates patient/family learning needs; teaches and documents appropriately.
- Delegates and supervises patient care; functions as charge nurse according to protocol.
- Assists nurse manager with performance appraisals.
- Participates in unit‑based QI projects and initiates unit‑specific projects related to clinical care.
- Manages patient care within assigned time frames; actively participates in care coordination.
- Willingly floats to other hospital areas.
- Promotes a culture of safety by fostering a culture of trust, participation, and collaboration.
- Identifies potentially dangerous situations, informs manager, and takes action to minimize risk.
- Reports errors and documents them appropriately.
- Performs other related duties as required.
- Minimum of 2 years experience in Med/Surg or Critical Care.
- Graduate of an accredited nursing school.
- Current New York State RN license.
- Certified in BLS, ACLS, PALS (and other specialty certifications as required).
- Experience anticipating contact with human blood and other infectious materials; adherence to universal precautions.
- Excellent customer service skills.
- Strong verbal and written communication skills.
- Ability to form effective working relationships with multidisciplinary team and all hospital personnel.
