Home Care RN Case Manager

Poughkeepsie, NY
Compensation: $100,000 - $125,000 + large sign-on bonus

- Provide in-home care primarily to geriatric patients
- Perform skilled nursing services including wound vacs, PleurX drains, INR draws, dressing changes, and more
- Conduct start-of-care admission visits and re-visits
- Educate and communicate with patients and family members regarding medical conditions and care plans
- Manage a manageable daily caseload of 4-5 patients per day Document clinical notes accurately within the EMR system
- Collaborate closely with interdisciplinary home care team members
- Associate Degree in Nursing (ADN)
- 1+ year of Registered Nurse experience
- Active New York State RN license
- BLS certification
- Prior home care experience strongly preferred
