Overview
LVN-Full Time PM (Healthcare)

Responsibilities- Administrative Functions:
Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility. - Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
- Periodically review the department's policies, procedure manuals, job descriptions, etc., and make recommendations for revisions. Meet with assigned nursing staff and support personnel in planning the shifts services, programs, and activities.
- Ensure that the Nursing Service Procedures Manual is current and reflects the day to day nursing procedures performed in this facility; ensure compliance by nursing service personnel.
- Prepare written and oral reports/recommendations concerning the activities of your shift as required.
- Cooperate with other resident services when coordinating nursing services to maintain the resident s total regimen of care.
- Participate in development, maintenance, and implementation of the facility s quality assurance program for the nursing service department; participate in facility surveys as requested.
- Periodically review the resident s written discharge plan and participate in updating it as required; assist in planning the nursing services portion of the discharge plan.
- Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required.
- Admit, transfer, and discharge residents as required; complete accident/incident reports as necessary.
- Maintain the Daily Census Report and submit to the Business Office; perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting as necessary.
- Protect resident information: do not disclose user IDs or passwords and report any violations; protect resident s protected health information and report suspected violations.
- Charting and Documentation: complete and file required recordkeeping forms/charts on admission, transfer, and discharge; encourage physicians to review treatment plans and sign orders/progress notes as established.
- Receive telephone orders from physicians and transcribe orders to resident charts and care plans as required; record nurses notes accurately and completely; report accidents/incidents and forward to Director as required.
- Follow procedures; record diet orders; communicate with Food Services; report discrepancies to Nurse Supervisor.
- Process transfer forms and document unauthorized discharges; perform routine charting in accordance with policies.
- Drug Administration Functions: prepare and administer medications as ordered; verify resident identity; maintain narcotic records; review medication cards for accuracy; notify physicians of auto stop orders; dispose of drugs and narcotics per procedures; order medications and supplies as needed.
- Notify the attending physician of any changes in resident condition; monitor and respond to emergencies as needed.
- Participate in employee performance evaluations and determine shift staffing needs; communicate with Nurse Supervisor about staffing and absences; evaluate department workforce and assign duties.
- Provide leadership to nursing personnel; perform daily rounds to ensure standards; address problem areas with Nurse Supervisor; ensure staff are trained and supervised properly.
- Ensure all nursing assistants are enrolled in or have completed an approved training program; supervise nurse aide trainees as required; foster a positive work environment and liaise with other departments to meet resident needs.
- Review complaints and grievances; follow established procedures; administer disciplinary actions fairly and without bias.
- Shift handoffs and occupational safety: conduct nursing handoffs at shift changes; report occupational exposures; ensure computer security according to policy.
- Inform nursing personnel of admissions and room assignments; greet and orient new residents and families; escort them to rooms as needed.
- Coordinate diagnostic and therapeutic services with physicians; review charts for treatments, medications, and diets; ensure treatments are administered properly.
- Supervise direct nursing care to ensure it aligns with care plans; coordinate social and activity programs with nursing services.
- Notify physicians of changes and involve family when appropriate; carry out restorative and rehabilitative programs; maintain clean and safe treatment areas.
