Position: Vancouver Coastal Health Outreach Worker

As a key member of the Community Mental Health and Substance Use team, your role involves providing holistic support to individuals facing mental health and substance use challenges. You will monitor clients, help them access various resources, and work collaboratively to ensure comprehensive care and well-being.

Key Responsibilities:

• Support individuals and families accessing opioid agonist therapy

• Monitor client conditions and arrange necessary health services

• Escort clients to appointments and community resources

• Administer medication and provide health-promoting information

• Conduct general clerical duties and participate in care planning

Requirements:

• Diploma in Community Social Services and one year related work

• CPR certification and a valid BC driver’s license

• Knowledge of decriminalization frameworks and harm reduction

• Proven ability to communicate effectively and work within teams

• Physical capability to perform essential outreach duties
