Vancouver Coastal Health (VCH) is hiring a Primary Care Nurse for the Coastal team in Powell River, BC. The RN will assess, plan, implement and evaluate nursing care for patients in a team-based Primary Care Network setting, collaborating with family physicians or nurse practitioners and other care team members.
Responsibilities include health promotion, chronic disease management, and coordinating transitions of care while upholding professional standards and the VCH values.
