Vancouver Coastal Health seeks a skilled Mental Health and Addictions Case Coordinator Nurse (RN/RPN) for the Complex Care Housing team. This role requires expertise in mental health assessments, treatment planning, and crisis intervention. You will collaborate within a multidisciplinary team to facilitate access to essential services while supporting clients and their families in a compassionate manner.

Key Responsibilities:

• Provide treatment services within a multidisciplinary team

• Conduct mental health assessments and treatment planning

• Facilitate crisis interventions and individual counseling

• Maintain relationships with clients and their support systems

• Coordinate access to treatment services based on best practices

Requirements:

• Diploma in Nursing or Psychiatric Nursing

• Two years of related experience in mental health services

• Registered Nurse or Registered Psychiatric Nurse in BC

• Valid BC Driver’s License for local travel

• Broad knowledge of mental illness, addiction treatment, and counseling
