Overview
Nurse Practitioner - PRN role rix Medical Network provides in-home clinical assessments and health services nationwide, focusing on personalized care and improving health outcomes for older adults and at-risk individuals.
Responsibilities- Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Administer and order point-of-care and lab-based testing as needed.
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary.
- Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities.
- Flexibility — Visits are scheduled based on your availability. Assessments can be conducted Sunday–Saturday from 8 am–7 pm, with scheduling options that fit day, evening, or weekend availability.
- Competitive pay — Earn income based on the number of assessments completed. Earning potential increases with availability and travel to see members outside your home location. Example ranges: $1,800–$11,500 per month based on 20–100 visits per month and a per-assessment rate of $90–$115 depending on state of residence.

- Education and support — Comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education.
- Additional licensure — Support for obtaining additional state licensure and credentialing in neighboring states or others you may visit while completing health assessments.
- Full-time (FT) opportunities — PRN providers can transition to FT roles and may receive a FT sign-on bonus.
- Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
- Current NP licensure in state of practice with prescription authority or ability to obtain prescriptive authority as needed.
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
- Current BLS, ACLS or CPR certification.
- 1 year of NP experience preferred; new grads encouraged to apply.
- Valid state driver’s license and proof of adequate automobile insurance coverage for the state of residence.
- Strong computer skills and familiarity with employee health/medical record software.
