**** Develop and update individualized care plans
* Provide in-home physical therapy services per physician’s plan of care
* Assess functional status, mobility, and home environment
* Educate patients and families on therapy techniques and equipment
* Maintain accurate documentation and communicate progress to physicians

* *** Licensed Physical Therapist in the State of Arizona
* Degree from a CAPTE-accredited program; passed state-approved exam
* Minimum 2 years’ experience (home health preferred)
* Current CPR certification
* Valid driver’s license, insurance, and reliable transportation
