Role:
Licensed Practical Nurse (LPN)

Location:

State owned assisted living facility in Prescott

Shift

Hours:

10:30pm - 7:00am

Requirements:

• Active LPN license issued by the state of Arizona
• Ability to obtain and maintain a fingerprint clearance card issued by the Arizona Department of Public Safety
• Valid driver's license (may be required for State duty)
• Compliance with vaccination requirements as per CMS Interim Final Rule - Medicare and Medicaid Programs;
  Omnibus COVID-19 Health Care Staff Vaccination (published November 5, 2021) (86 FR 61555)
• Current negative TB skin test (Mantoux skin test) within the last 6 months OR a written statement from a physician, physician's assistant, or a registered nurse practitioner indicating freedom from tuberculosis (if previously tested positive)

Pay: $44/hr per hour
