Role specifics and benefits:
In this Home Health RN role, you’ll perform comprehensive in-home nursing care for medically complex patients, including assessment, care planning in collaboration with physicians and the care team, wound and IV therapy management, medication reconciliation and administration within scope, pain control, palliative support, and preventive teaching for patients and family caregivers. You’ll document care plans and progress in electronic health records, monitor patient status, recognize red flags, and coordinate with pharmacies, durable medical equipment suppliers, social workers, and therapy teams to ensure a seamless continuum of care.
You’ll educate patients and families about disease management, medication safety, fall prevention, nutrition, and lifestyle modifications, empowering them to participate actively in their recovery and managing chronic conditions.
