Part-time or Full Time Performer Dentist required for 2-5 days per week (FLEXIBLE) in a busy, mixed, well-established, INDEPENDENTLY OWNED practice near Preston, Lancashire

£13 per UDA
GOOD PRIVATE POTENTIAL
5000 UDAs available flexible
50% - 50% split on Private & Labs
Existing well maintained list
Fully computerised surgeries - 2 surgery practice, Dental Plus
Digital x-rays, intra-oral Scanner, Rotary Endo,
Full, longstanding support team including a Therapist
Parking on site
30 minutes to Blackburn, and Southport, 35 minutes to Blackpool
Available immediately, start date negotiable

Compliance requirements:

Full NHS Performer Number if applicable
Local Area Team confirmation
GDC certificate
UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
Indemnity certificate
Passport
Enhanced DBS (within the last 12-36 months)
Clinical references covering places of employment over past 2 years (minimum 2 referees)
Degree certificate
CPR/AED/Medical Emergencies certificate dated within the last 12 months
