CHC Advisor - Band 6

£19.38 - £31.01 per hour

Job Overview

Bluestones Medical is currently seeking an experienced and motivated CHC Advisor (Band
6) to join the CHC & IPA Commissioning Team supporting the delivery of Continuing Healthcare services across the Lancashire & South Cumbria region.

The successful candidate will play a vital role in supporting the All Age Continuing Healthcare (CHC) service, ensuring high-quality, patient-focused care in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.

This role requires flexibility to work across the Lancashire & South Cumbria ICB footprint and involves close collaboration with multidisciplinary teams, service providers, safeguarding teams, local authorities, and other stakeholders.

A full UK driving licence and access to a vehicle are essential for this position.

Key Responsibilities

Clinical & Case Management
  • Support and facilitate multidisciplinary assessments and patient reviews in line with CHC National Framework guidance.
  • Manage and coordinate CHC-funded care packages, ensuring appropriate, effective, and high-quality care delivery.
  • Monitor patient care arrangements and contribute to future commissioning decisions and resource planning.
  • Maintain accurate and contemporaneous clinical records in accordance with professional standards and organisational policies.
  • Act as a first point of contact for internal and external enquiries relating to individual CHC cases.
  • Provide specialist advice on clinical and commissioning issues where appropriate.
  • Ensure compliance with service agreements, safeguarding procedures, and commissioning policies.
Partnership & Stakeholder Working
  • Work collaboratively with:
    • Continuing Healthcare teams
    • Safeguarding teams
    • Local Authorities
    • Care providers and NHS Trusts
    • Service users, families, and legal representatives
    • Finance and Quality Monitoring teams
  • Attend review meetings, complex case discussions, and Court of Protection hearings where required.
  • Develop and maintain effective relationships with internal and external stakeholders.
Service Improvement & Governance
  • Support service development initiatives and contribute to continuous improvement programmes.
  • Assist with quality monitoring activities and implementation of governance strategies.
  • Contribute to operational planning, commissioning strategies, and performance improvement initiatives.
  • Ensure safeguarding responsibilities are upheld in line with ICB policies and procedures.
  • Maintain compliance with NMC/AHP professional standards, revalidation requirements, and mandatory training.
Information & Data Management
  • Analyse and interpret complex clinical and commissioning information.
  • Maintain and update case management systems and databases accurately.
  • Prepare reports, briefings, and correspondence for senior managers and stakeholders.
  • Monitor risks, issues, and performance indicators, escalating concerns appropriately.
Leadership & Team Support
  • Support induction, training, and supervision of staff where required.
  • Participate in recruitment and team development activities.
  • Provide specialist advice and guidance to colleagues and stakeholders.
Essential Requirements
  • Registered healthcare professional (NMC/AHP registered).
  • Experience working within Continuing Healthcare (CHC), case management, or commissioning environments.
  • Strong knowledge of the National Framework for NHS Continuing Healthcare and Funded Nursing Care.
  • Understanding of safeguarding, MCA, and DoLS legislation.
  • Excellent communication, organisational, and analytical skills.
  • Ability to manage complex caseloads and work independently.
  • Full UK driving licence and access to a vehicle.
Desirable
  • Experience with in commissioning or integrated care settings.
  • Knowledge of Court of Protection processes.
  • Experience in quality monitoring and service improvement initiatives.
