What you will do
  • Rehabilitate patients at home following their hospital discharge, ensuring a smooth transition and ongoing support.
  • Conduct comprehensive assessments for aids, equipment, and adaptations to meet each patient’s unique needs.
  • Perform moving and handling assessments to ensure patient safety and promote independence.
  • Work closely with the multi-disciplinary team to develop personalised care plans.
  • Provide ongoing support and education to patients and their families for optimal rehabilitation outcomes.
Requirements
  • Must be registered with HCPC with a minimum of 2 years’ experience in a relevant specialism.
  • Being a car driver is preferable to facilitate home visits and ensure timely service delivery.
