Community Roots Facilitator

Our Vision is to provide the best mental health, learning disability, autism, community, and physical health services for the populations we serve.

As an integrated Trust, Lancashire and South Cumbria NHS Foundation Trust (LSCFT) delivers a comprehensive range of services, including:

• Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
• Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
• Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long-term condition management.

The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health, general nurses, children’s nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers, as well as those specialising in learning disabilities and community physical health services.

An exciting opportunity has arisen for a motivated, enthusiastic and empathic people person to join our Community Roots service, working in the Central, West & Fylde localities. You will be customer focussed with a passion for engaging with the community and helping people access a wide range of health and wellbeing opportunities.

Are you passionate about mental health, community engagement, and inclusive learning? Join our Community Roots team to co-design and deliver transformative mental health programmes across diverse communities. This role is pivotal in promoting recovery, resilience, and reducing stigma through workshops, signposting clinics, and collaborative learning.

Applications are encouraged from individuals with demonstrable experience in working with communities and mental health care services.

At Community Roots we offer courses that are co-designed, alongside people with lived experience and professionals by experience, to help people find accessible and fun health and wellbeing opportunities important to them.

We work alongside a range of community partners to bring the very best opportunities in your local area. We want to help people make connections in their community and we're able to signpost to support when needed. By working with a range of different partners, we hope to bring opportunities that are relevant to our community and people’s recovery journeys.

Co-produce curriculum content with experts by experience and professionals

Evaluate and improve course quality based on feedback and developments

Represent the Community Roots team and contribute to service development
