Care Co-Ordinator

An exciting opportunity has arisen for a highly motivated individual to join our friendly team as a Care Co-Ordinator. If you are looking for a part-time role in a friendly rural location, we may have the ideal position for you. The position is for 20 hours per week, days and times to be confirmed at interview., The introduction of Care Co-ordinators within the Wyre Bridge PCN is a significant opportunity.

This is a role which will evolve along with new functions but will involve working with our practice team and wider multidisciplinary teams as a pivotal role in ensuring all patients receive the best possible care and services available.

You will be responsible for helping to plan and co-ordinate the care needs of our most complex patients.

This role will support the practice in the coordination and delivery of key PCN DES delivery and supporting digital initiatives and responsibilities for ensuring the patient journey through primary care and care transfer across the health system is seamless.

Great Eccleston health Centre is a rural GP practice with an excellent reputation for patient care and service provision.

The Practice is part of a two Practice Primary Care Network with Over Wyre Medical Centre known as Wyre Bridge PCN.
