Practice Nurse

Kirkham Health Centre, a GP surgery serving around 9,800 patients across mixed urban and rural areas, is recruiting a Practice Nurse. The work centres on managing long-term conditions, cervical screening, immunisations, wound care and routine nursing treatments, alongside GPs and the wider primary care team. The post sits within a practice that also employs several GPs, advanced nurse practitioners and a nursing associate.

Registration with the Nursing and Midwifery Council is required, and experience of a fast-paced healthcare setting is expected.

• Managing chronic disease and supporting patients with long-term conditions
• Childhood and adult immunisations and injections
• Cervical screening and cytology
• Wound care and management
• Health promotion
• Routine nursing treatments
• Providing health education and advice
• Contraceptive advice
• Undertaking clinical assessments
• Maintaining accurate patient records
• Assisting with QOF and audits
• Working with GPs and the multidisciplinary team to deliver patient-centred care

• Evidence of continuing professional development

• Registration with the Nursing and Midwifery Council

• History of positive working relationships
• Experience of working in a fast-paced healthcare setting

• Proven experience as a Practice Nurse in primary care
• Experience of using EMIS
• Postgraduate degree or diploma in chronic disease management
