HCRG Care Group is recruiting a Healthy Family Practitioner in Preston to support children, young people and families across Lancashire. This part-time fixed-term 30 hours per week offers the chance to work within the Young People & Healthy Families Service, collaborating with Health Visitors and School Nurses to deliver compassionate, high-quality community care.
The role focuses on delivering the Healthy Child Programme and targeted family interventions, with a planned start date of 01/09/26
