Overview

Healthy Family Practitioner based in Preston. Part-time fixed-term opportunity working 30 hours per week with a planned start date of 01/09/26 to 31/08/27. Based in the Young People & Healthy Families Service, you’ll collaborate with Health Visitors, School Nurses, and multidisciplinary teams to support children, young people, and families across Lancashire and help improve long-term health outcomes.

This is a rewarding community-based role that empowers families, promotes child development, and delivers high-quality, compassionate care.

Main duties
  • Deliver elements of the Healthy Child Programme, including developmental reviews (e.g., 1-year and 2.5-year checks).
  • Support children and families with targeted interventions such as behaviour management and infant feeding.
  • Run community clinics and Healthy Family drop-in sessions.
  • Work directly with families in community settings, including home visits.
  • Build strong relationships with nurseries and local services.
  • Identify safeguarding concerns and escalate appropriately.
  • Encourage service uptake and advocate for families’ needs and voices.
  • Maintain accurate records and contribute to service improvement.
Qualifications
  • NNEB or equivalent (e.g., BTEC National Diploma in Nursery Nursing, NVQ Level 3 Childcare & Education, Foundation Degree in Early Years/Education).
Essential Experience & Skills
  • Experience in health promotion or public health activities.
  • Team working experience.
  • Aware of safeguarding and child protection.
  • Strong understanding of child development and parenting.
  • Effective written and verbal communication.
  • Ability to work independently within a multidisciplinary team.
  • Accurate record keeping and high standards of documentation.
  • Ability to contribute to audit and research.
  • Ability to create and implement family-centered action plans.
  • IT literacy, including local and web-based systems.
  • Commitment to continuous learning and development.
  • Understanding of clinical governance.
  • Full UK driving licence and access to a vehicle.
