Contract:Full Time | Ongoing Contract
Working Pattern:Monday to Friday with weekend rota as required

We are looking for an experienced
Brokerage Officer
to join Lancashire's Brokerage Service, supporting adults and young people preparing for adulthood.

This is a rewarding role where you'll work collaboratively with individuals, families, social workers and providers to source high-quality, person-centred care and support that promotes independence, delivers value for money and meets assessed needs.

About the Role

As a Brokerage Officer, you will arrange and commission care and support packages for adults aged 18+ and young people transitioning into adult services. Working in partnership with professionals, providers and families, you'll identify the most appropriate support using approved provider frameworks, block contracts and community resources.

You'll play a vital role in ensuring care packages are sourced quickly, safely and within budget, helping people remain independent and achieve the outcomes that matter most to them.

The service also supports urgent and same-day placements, hospital discharges and seasonal demand, requiring a flexible and solution-focused approach.

Key Responsibilities
  • Source and arrange person-centred care and support packages for adults and young people preparing for adulthood.
  • Work closely with individuals, families, social workers and partner agencies to co-produce support plans.
  • Identify cost-effective services that promote independence, wellbeing and positive outcomes.
  • Maximise the use of local community resources, low-cost services and existing support networks where appropriate.
  • Negotiate with care providers to secure quality placements and support within agreed budgets.
  • Arrange residential, nursing, supported living, domiciliary care and specialist placements where required.
  • Utilise framework agreements, approved provider lists and block contracts effectively.
  • Support timely hospital discharges and urgent care arrangements.
  • Maintain accurate records and ensure services are commissioned promptly and safely.
  • Work collaboratively across the wider Brokerage Team to respond to changing service demands.
About You

We're looking for someone who has:

  • Experience working within adult social care, brokerage, commissioning or care coordination.
  • Knowledge of care packages, domiciliary care, residential care, supported living and community-based services.
  • Experience sourcing placements or negotiating with care providers.
  • An understanding of strengths-based and person-centred practice.
  • Excellent organisational, negotiation and communication skills.
  • The ability to manage competing priorities and work effectively under pressure.
  • Experience working collaboratively with social workers, health professionals and external providers.
  • A commitment to delivering high-quality, cost-effective services that improve outcomes for people.
