Position

An exciting opportunity has arisen for a Band 6 Assessment & Rehab Nurse within our South Ayrshire Intermediate Care Team based at Biggart Hospital.

Hours

From 1 April 2026, the working week for NHS Agenda for Change staff in Scotland will be reduced. Full-time hours will reduce from 37 to 36 hours (pro rata for part-time staff) without loss of earnings. Hours of duty: 37 hours per week over 7 days. This post is offered on a permanent basis. Part-time hours may also be considered for this post.

UK Driving Licence:
Full UK driving licence and access to a vehicle is essential to this role.

What You Will Do

Work as an autonomous practitioner in the community to provide person-centred specialised, rapid assessment and care for patients with a diverse range of complex presentations and multiple co-morbidities. Supporting the delivery of rapid response time line, prevention of inappropriate admission to hospital and facilitation of early supported discharge. You should be able to demonstrate competencies for care and rehabilitation of people with complex needs.

  • Treat and manage a large and complex clinical caseload.
  • Provide a wide and varied range of healthcare options to a diverse range of clinical conditions and patients with physical, psychological, mental and social needs in a community setting, ensuring the highest standards of patient care.
  • Participate in service development for the team within the Health & Social Care Partnership as we head towards an enhanced level of support for patients in our care.
  • Be required to coordinate day to day work and allocation of work to others. Deal with operational issues as they arise. Allocation of work will be to multi-disciplinary staff of differing banding and reablement care staff.

Knowledge, Training And/or Experience Required To Do The Job

A Diploma/Degree in Registered General Nursing, with experience working within a multi-disciplinary Team, good written/verbal communication skills and an excellent team worker are essential. Evidence of continuing professional development, including extensive experience in relevant clinical field in health and/social care. The post holder will be required to have basic information technology skills.
