Case Manager

Our client is seeking a dedicated Case Manager to join their community services team in Pretoria . In this crucial role, you will be responsible for coordinating and facilitating comprehensive care plans for individuals and families facing complex needs. You will act as a central point of contact, connecting clients with essential resources, advocating for their needs, and monitoring their progress towards achieving stability and independence.

This hybrid position allows for both essential fieldwork within the community and remote administrative duties, providing a balanced approach to client service delivery in the Gauteng region.

• Conduct thorough assessments to determine client eligibility and service needs.
• Develop and implement individualized case management plans in collaboration with clients.
• Coordinate services with various community agencies, healthcare providers, and support networks.
• Monitor client progress, making adjustments to service plans as necessary.
• Maintain detailed and accurate case notes, reports, and client files.
• Provide advocacy and support to ensure clients receive appropriate services and resources.

• Bachelor's degree in Social Work, Psychology, or a related field.
• Minimum of 3 years of experience in case management or a similar client-facing role.
• Strong understanding of social services systems and available community resources in Pretoria .
• Excellent organizational, time management, and problem-solving skills.
• Proficiency in using case management software and standard office productivity tools.
