Registered Practical Nurse - Transitional Care Program

- Department: UHN Reactivation Care Centre (Transitional Care Program)
- Compensation: CAD $38.42 – 39.02 per hour
- Hours:
15 per week - Shifts:
07:15‑19:15, 19:15‑07:15 – rotating days, nights, weekends, holidays (subject to change) - Reports to:

Position Summary
As a member of a multi‑disciplinary team, the Registered Practical Nurse practices in accordance with relevant legislation, the UHN Professional Practice Model, and the mission and vision of nursing at UHN to provide compassionate, collaborative and competent care while meeting legal and ethical requirements.
Duties- Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in an interdisciplinary setting.
- Use current theoretical and research knowledge and input of team members to plan and implement care.
- Participate in inter‑professional team care planning forums, inter‑professional rounds and family conferences.
- Provide mentorship to RPN students and new staff on the unit.
- Participate in professional, program and hospital‑wide initiatives, including professional development.
- Comply with patient safety responsibilities and corresponding hospital plans, policies and procedures to ensure a safe environment for patients.
- Valid registration with the College of Nurses of Ontario.
- Diploma in Practical Nursing.
- Recent related clinical experience preferred.
- Effective problem recognition, problem solving, and decision‑making skills.
- Commitment to continuing professional development.
- Commitment to team success.
- Resilience and effective coping strategies to manage change.
- Computer literacy in word processing and Electronic Patient Record.
- Working knowledge of the College of Nurses of Ontario standards of practice.
- Evidence of satisfactory attendance and punctuality.
