Helps clients and families navigate services, appointments, and transitions of care-removing barriers to access. Nurse Navigators educate, coordinate care, and advocate for culturally safe care, often in complex pathways (e.g., oncology).

• Assess needs and coordinate care plans.
• Book referrals and support transitions.
• Provide education on conditions and services.
• Advocate for culturally safe care.

• Grade 12 diploma — Regular Grade 12, Adult 12, or GED.
