Nurse Practitioner

Position: Full-Time Nurse Practitioner
Join Northern Health as a full-time Nurse Practitioner, providing innovative care through the Hospital at Home program in Prince George, British Columbia. This role focuses on delivering acute care to patients within their own homes.

As a vital member of an interprofessional team, you will coordinate patient care and utilize virtual monitoring tools.

Experience with Power Chart EMR will ensure effective documentation and seamless care transitions. You will work in rotating 12-hour shifts to ensure ongoing support for patients' medical needs.

Key Responsibilities:

• Offer acute home-based care for adult patients

• Coordinate care as the MRP for assigned patients

• Collaborate with a diverse healthcare team

• Document and manage care via Power Chart EMR

• Participate in shift rotations for continuous patient coverage

Requirements:

• Current Nurse Practitioner certification required

• Previous experience in acute care is beneficial

• Strong documentation and communication skills

• Enthusiasm for innovative care models

• Commitment to professional development
