LPN Primary Care Nurse

Position: LPN Primary Care Nurse Needed
Become a vital member of Highland Health & Family Development Centre as a Primary Care Nurse. Deliver effective patient care and supervise a dedicated team in community health.
In the role of Licensed Practical Nurse, you will lead the Home Support Services team, overseeing Community Health Workers while implementing care plans. Your responsibilities will include the administration of medications and training staff in best practices. A current registration with the BC College of Nurses and Midwives and two years of experience, including in a supervisory capacity, are essential for this position.

Key Responsibilities:

• Coordinate care plans with Community Health Workers

• Perform nursing assessments and personal care deliveries

• Manage medication administration per guidelines

• Train and evaluate staff in care practices

• Apply best practices in a collaborative healthcare environment
Requirements:

• Registered as an LPN with BC College of Nurses and Midwives

• Minimum of two years' experience, including supervisory

• Valid BC Driver's License required

• Comprehensive understanding of clinical specialties

• Capacity to work independently and with teams
