Nurse Practitioner - Hospital at Home Program

In Prince George, Northern Health is looking for four dedicated Nurse Practitioners for a full-time position. You will function as the Most Responsible Provider, collaborating with a dedicated interprofessional team while ensuring continuity of care through virtual support. Bring your clinical expertise to this innovative program that supports safe, effective at-home treatment.

Key Responsibilities:

• Deliver in-home acute care to adult patients

• Serve as the MRP for assigned cases

• Engage with an interprofessional healthcare team

• Document care accurately through Power Chart EMR

• Participate in rotating 12-hour shifts for patient coverage

Requirements:

• Master’s degree in nurse practitioner from an accredited program

• Eligible for registration with BCCNM

• Effective communicator with patients and healthcare teams

• Proven ability to evaluate health research literature

• Experience in health promotion and preventive strategies
