Key Responsibilities:

• Deliver direct clinical care to individuals and groups

• Support families in community settings such as homes and schools

• Engage in clinical follow-up aligned with personal health goals

Requirements:

• Registered Nurse or Licensed Practical Nurse certification

• Strong commitment to patient-centered care

• Ability to work as part of an interdisciplinary team

• Knowledge of community health resources

• Availability for Monday to Friday shifts 08:00 to 16:00
