Jordan’s Principle Service Coordinator
Location: Burns Lake, Vanderhoof or Prince George
Employment Type: Full Time – 35 hours per week

The Opportunity:
- Serves as a point of contact for (a) Indigenous children and their families in the CSFS service delivery area (b) health care providers, and (c) First Nations communities and organizations who are requesting support and advocacy directly related to children and youth.
- Ensures the appropriate and effective sharing of information including identifying and maintaining contact with Indigenous children and families as required for continuity of care, including the use of multiple communication strategies including telehealth.
- Collaborates with a multi-disciplinary team to provide a timely response when children are in need of care.
- Develops and documents a coordinated service plan that includes referral, assessment, treatment/admission, discharge (if applicable), transition and ongoing care protocols.
- Bachelor’s degree in a health or social services program or related discipline or related field of study preferred
- A diploma in a Human Services related field required
- Three (3) years’ recent related experience working in health and social services, particularly with Indigenous Peoples.
Wage Range: The target hiring range for this position is $38.0337 to $46.7918 per hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range. Placement within the wage range is dependent on the applicants’ role specific knowledge, skills, abilities, related experience and in consideration of factors such as market conditions and internal equity.
