Join Carrier Sekani Family Services as a full-time Jordan’s Principle Service Coordinator. Engage with Indigenous families and health providers to foster a culture-centered approach to care delivery.
This full-time role is essential for connecting Indigenous children and families with the comprehensive services they require. You will collaborate with a multi-disciplinary team to ensure timely responses for those in need, develop coordinated service plans, and utilize diverse communication strategies, including telehealth. A commitment to health and social services is vital in this position.

Key Responsibilities:

• Act as a contact point for Indigenous families and health providers

• Ensure effective sharing of information for continuity of care

• Collaborate with a multi-disciplinary team for timely support

• Develop service plans covering assessment, treatment, and ongoing care

• Maintain contact with families to assess ongoing needs
Requirements:

• Diploma in Human Services required

• Bachelor’s degree in related field preferred

• Three years’ experience in health and social services

• Experience working with Indigenous Peoples

• Strong communication and collaboration skills
