Position: Health Service Coordinator for Indigenous Youth
Elevate Indigenous health initiatives as the Jordan’s Principle Service Coordinator at Carrier Sekani Family Services. Engage with children and families in northern BC through a full-time position.

In this role, you will be a key point of contact for Indigenous families in need of support and advocacy. Collaborating with multidisciplinary teams, your focus will be on ensuring timely access to health services. You will develop coordinated service plans to enhance care continuity for children and youth in the community.

Key Responsibilities:

• Serve as the main contact for Indigenous families needing support

• Ensure effective communication among stakeholders for care access

• Collaborate on care responses with health professionals

• Develop and document individualized service plans

• Advocate for children's health needs in First Nations communities

Requirements:

• Bachelor’s degree in health or related field preferred

• Diploma in Human Services required

• Minimum of three years in health and social services

• Understanding of Indigenous health and social issues

• Excellent organizational and interpersonal skills
