Under the direction of the Home & Community Care Manager, your role will include supervising support staff, performing nursing procedures, and educating clients on health management. Your expertise will help ensure high-quality care for the frail elderly and disabled.

Key Responsibilities:

• Plan and coordinate care in community settings

• Conduct comprehensive assessments for clients

• Direct and support community health and residential aides

• Provide health education to clientele

• Implement individualized care plans based on assessments

Requirements:

• Practicing LPN registrant in BC

• Minimum two years of acute care experience

• Class V BC Driver's License

• Valid CPR and First Aid certificates

• Experience working with the frail or disabled
