As an Independent Optometrist leasing space inside Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being. You will have autonomy to establish your days of practice, hours of operation, patient schedule, fee schedule, and keep 100% of exam/professional fees.
Responsibilities- Launch services in Walmart stores.
- Provide eye and vision examinations.
- Maintain patient records and ensure follow‑up care.
- Comply with state regulations and Walmart policies.
- Full‑time schedule.
- Fair Market Value lease option for equipment and administrative support (if state allows).
- Keep 100% of exam/professional fees.
- Doctor of Optometry (OD) from an accredited optometry school.
- Valid state optometry license.
- General Liability Certificate.
- Professional Liability Insurance Certificate.
- W-9 form.
- Completed lease agreement terms.
- Ophthalmologist (MD or DO) from an accredited school.
- Valid state ophthalmology license.
