Physical Therapist, Home Health – Center Well Home Health

• Plan and administer skilled physical therapy treatment for patients with various injuries, illnesses, and functional disabilities to achieve the highest level of physical function.
• Test/screen patients’ physical strengths to assist the physician in evaluating the level of function, document findings, and develop or pursue treatment programs with measurable objectives.
• Develop and implement conditioning/rehabilitation programs aligned with the physician’s Plan of Treatment and overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
• Confer with the physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
• Provide written instructions/care plans to Physical Therapy Assistants and Home Health Aide staff, supervise performance, and monitor documentation completion as part of supervisory responsibilities.
• Accurately and promptly document patients’ care observations, interventions, and evaluations; ensure interim orders are properly recorded, submitted for signature, and implemented.
• Report patient progress to the physician, Clinical Manager, staff, patient, and family. Submit evaluation, treatment plans, progress reports, and discharge summaries to the supervisor and care management staff.

• Degree from an accredited Physical Therapy Program approved by the APTA.
• Minimum of one year of physical therapy experience (preferred).
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.

Base pay range: $85,400.00 – $ per year.

Pay per visit/unit: $54.00 – $76.00.

Scheduled weekly hours: 40.
