Case Manager role at Encompass Health
. This is a part-time position with responsibilities spanning from admission to post-discharge coordination within our interdisciplinary team.

$30.00/hr - $43.00/hr

• Manage resources and coordinate patient care from admission to post-discharge.
• Oversee interdisciplinary plan-of-care decisions and ensure timely, appropriate discharge planning.
• Collaborate with the interdisciplinary team to support patient and family goals and outcomes.
• Attend meetings, participate in utilization review processes, and compile/maintain case management documentation.
• Monitor compliance with regulations, payor requirements, and contract levels; make timely referrals to post-discharge providers as needed.
• Educate patients and families about rehabilitation and the Case Manager role; establish and execute a communication plan.
• Schedule and facilitate family conferences; assist with procuring resources to prevent discharge delays.
• Support ongoing monitoring of patient experience and quality indicators.

• Licensed or certified professional appropriate to the discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counselor) with ability to perform independent assessments where applicable.
• Active license if required by state for the specific discipline.
• CCM® or ACM certification eligibility at entry or within two years of entry; CCM/ACM certification required or must be obtained within two years for some positions.
• Minimum education:
  
  bachelor’s degree for licensed/certified health professionals; nursing track requires Associate Degree in Nursing (BSN preferred); advanced degrees preferred for non-nursing roles.
• Rehabilitation experience preferred (minimum 2 years).
