JOB SUMMARY: The Mental Health Clinician & ACT Team Lead functions as the Lead Clinician for the Assertive Community Treatment Team for Crook County. This is a multi-disciplinary team of behavioral health professionals to provide or coordinate rehabilitation and community support services for clients who are recovering from severe mental health conditions (e.g., high risk for hospitalization, being jailed, or placed in some other restrictive facility as a result of or in conjunction with their mental illness).
Responsibilities- Provides clinical guidance and supervision to the members of the ACT team regarding client care, and completes team scheduling and assignments to ensure balanced caseloads and draw on the strengths of each team member;
- Provides community outreach to the most at-risk and mentally ill clients in the community;
- Consults with professionals on a variety of problems including delivery of needed services for clinically complex cases;
- Completes intakes, assessments, and treatment plans as appropriate;
- Identifies precipitating events, gathers histories of mental, emotional and physical disabilities, alcohol and drug use and substance abuse disorders, past mental health services and criminal justice contacts; assesses family, social, and work relationships; conducts mental status assessments; documents a multiaxial DSM diagnosis; writes and implements or supervises implementation of a treatment plan; conduct and documents mental health assessments; and provide mental health treatment and rehabilitative services within the scope of their practice;
- Works closely with other staff as the Lead for the multi-disciplinary ACT treatment team and in collaboration with other agencies to provide individual, group, family counseling and case management achieve the best outcomes for the clients;
- Completes training required to perform mental health crisis work and participates as part of the MH crisis rotation;
- Develops and maintains strong collaborative relationships with a wide range of community partners, including (but not limited to):
Crook County Jail, Crook County Juvenile Department, Crook County Adult Community Justice, Crook County DHS and Public Health departments, and other programs and departments; - Works closely with referral sources, keeping them informed on the progress of the client, as needed and appropriate to client confidentiality and HIPAA;
Provides individualized services, including:
Individualized assessment and treatment planning; case management, crisis intervention; individual supportive therapy; medication prescription monitoring and delivery; substance abuse services; work-related services; support for Activities for Daily Living (ADLs); social, interpersonal relationship, and leisure-time skill training; education, support, and consultation to consumers’ families and other supports; coordination of
- hospital admissions and discharges; other support services; includes being a member of the mental health crisis rotation team;
- Leads groups for psycho-educational, skill development, anger management, parenting, process, gender-specific, family, and/or other groups as assigned;
- Maintains accurate documentation of all services in client files in EHR within agency-stated timelines and in accordance with regulations and Best Care standards and practices;
- Complete clinical chart reviews for the ACT Team;
- Models appropriate counselor behavior to other staff; helps train and orient new staff to the program as necessary;
