Advanced Practice Provider, under the general supervision of the Clinical Division Director and direct supervision of a designated physician, provides medical and appropriate outpatient care to patients of all ages. Care includes evaluation, diagnosis, treatment, documentation, and coordination of medical, ancillary, nursing, and other services within a Medical Home model to achieve best and most cost‑effective outcomes. Treatment plans incorporate behavioral health, RN care management, therapy, pharmacy, nutrition or surgery to manage patient illnesses and injuries.
The APP monitors patients’ ongoing care plans and refers to specialists as needed.
APPs work a minimum of 40 hours per week with at least 36 hours of patient contact. Scheduled days vary with clinic operating hours. APPs participate in an equal share call rotation for evenings, weekends, and holidays, coordinated with other providers. A Family Care physician provides backup and expertise support as needed.

Duties and Functions Clinical Duties- Practice excellence in diagnosis, treatment planning aligned with patient values, and preventive care.
- Provide evidence‑based family medicine within scope of license, training, and credentialing.
- Collaborate with SCHS providers, specialty teams, and referring providers to ensure world‑class patient experience.
- Communicate therapeutically, listen actively, and use skillful communication techniques.
- Prescribe medications per SCHS policies and procedures.
- Consult with pharmacists for poly‑pharmacy and psychiatrists for psychotropic medications.
- Accurately document, code, and bill patient care in EHR or appropriate forms timely and in compliance with standards.
- Interpret and report diagnostic tests timely, within 72 hours for urgent and 7 days for routine diagnostics.
- Educate patients and families about preventive care and medical treatment use.
- Participate in clinic after‑hours call duties and responsibilities.
- Support delivery and improvement of quality health measures identified by SCHG.
- Consult with physicians as needed and have chart notes reviewed or approved.
- Lead and support departmental goals, vision, mission, and values of St. Charles Health System.
- Provide timely, efficient, and accurate customer service.
- Create a healing environment that respects the wholeness of each individual.
- Champion Value Improvement Practice (VIP‑Lean) principles and assist with policy formation.
- Cultivate intentional relationships, focusing on behaviors that enhance the care experience.
- Maintain billing and coding documentation within assigned time.
- Act with professionalism and confidentiality.
- Comply with all applicable laws, regulations, policies, and procedures, supporting the health system’s corporate integrity.
- Keep all licenses and certifications current and unrestricted.
- Enhance collaboration throughout the medical community.
- Maintain compliance with hospital Medical Staff Bylaws, Rules, and Regulations as required by SCHS.
- Master of Science in Physician Assistant Studies or Master of Science in Nursing.
- Completion of annual fire/safety and required educational in‑services.
- Attendance of mandatory educational offerings and regulatory required trainings.
- Completion of annual CME to maintain license and board certification.
- Completion of assigned Computer Based Learning modules on time.
- Doctorate of Science in Physician Assistant Studies or Doctorate of Nursing Practice.
- Unrestricted Physician Assistant and/or Nurse Practitioner license.
- Physician Assistant requires NCCPA certification;
