Family Care APP (NP or PA)

The Advanced Practice Provider (APP) provides comprehensive outpatient family medicine care under supervision, including evaluation, diagnosis, treatment, and coordination of services within a Medical Home model.

• Works at the Prineville Clinic, with a schedule of at least 40 hours/week, including patient contact and shared call duties.
• Collaborates with a multidisciplinary team to deliver personalized, evidence‑based care across all ages, incorporating behavioral health, pharmacy, and other specialties.
• Prescribes medications, interprets diagnostics, documents encounters, and educates patients on health management.
• Participates in quality improvement, clinic operations, and after‑hours call rotations.

• Requires a Master’s in Nursing or Physician Assistant Studies, current unrestricted licensure, relevant certifications, and outpatient clinical experience.
• Preferred qualifications include additional advanced degrees, Oregon licensure, and experience with Electronic Health Records and Medical Home models.

Compensation ranges from $154,568 to $165,351 annually, with benefits such as health insurance, retirement plans, and paid time off. Relocation to Prineville, OR, is required before starting.
