Physician Assistant – Assisted Living Visits

Overview

Twin Cities Physicians is seeking a Physician Assistant (PA-C) to join our care team in Golden Valley and across the Twin Cities Metro. In this role, you’ll provide on-site, high-impact care to older adults in assisted living, memory care, and other senior housing settings.
This is not a clinic-based job—it’s a flexible, autonomous role where you build real relationships with patients.

• Drive to senior living communities to provide on-site primary care to residents
• Facilitate the patient’s plan of care: chronic care management, advanced treatment, and end-of-life planning
• Coordinate with the interdisciplinary team, reconcile medications, and order labs
• Communicate clearly and regularly with physicians, support staff, facility teams, and families

• Current registration as a Physician Assistant / PA by the MN Board of Medical Practice
• Strong portfolio in geriatrics, primary care, or internal medicine
• Passion for serving the older adult population
• Strong interpersonal communication skills
• Valid driver’s license and current auto insurance
