Hope Health Hospice & Palliative Care is seeking an experienced Registered Nurse (RN) Case Manager to provide compassionate care to hospice patients in their homes across Northern Rhode Island. The RN will oversee clinical care, advocate for patients, and lead the care team with urgency and precision.
The role requires 1–3 years of clinical experience, RI (and MA) licensure, and strong assessment/communication skills. Work schedule is 40 hours weekly, with 8am-4:30pm hours and rotating weekends.
