Revive Therapeutic Services is seeking a Case Manager in Providence, RI. This full-time, hybrid role (4 days onsite, 1 day remote) operates Monday through Friday, 9am–5pm, serving elders with coordinated care plans and access to long-term support services.
You will maintain records, conduct assessments, and monitor progress, ensuring clients receive appropriate resources and guidance to enhance overall wellness. Mileage is reimbursed for RI travel.
