As an independent lease holder you will take a leading role in patient care, launching your services in Walmart stores. You will have autonomy over practice operations.
Responsibilities- Establish your days of practice
- Establish your hours of operation
- Control your patient schedule
- Set your fee schedule and keep 100% of exam/professional fees
- Doctor of Optometry (OD) from an accredited optometry school
- Valid state optometry license
- General Liability Certificate
- Professional Liability Insurance Certificate
- W-9
- Lease agreements provided by Gumberg Asset Management Company (ME & VA license agreements)
