JOB DESCRIPTION :
Submission Requirements :ALL OF THE FOLLOWING SHOULD BE NOTED IN ADDITIONAL SUBMISSION DETAILS. REQUIRED PRIOR TO CLIENT PRESENTATION UNLESS OTHERWISE NOTED
- Board Eligible (within 5 years of residency) or Board Certified in Urology
- REQUIRED - Active CO License (or willingness to obtain) - (note in "Additional Submission Details" field)
- REQUIRED - BLS and ACLS
- REQUIRED - Most recent NPDB Self Query Report (within 30 days active)
- REQUIRED no exemption, required at presentation - Visa Support: J1 or H1B candidates are eligible for visa sponsorship. (note in "Additional Submission Details" field) -
REQUIRED - DEA (must be obtained prior to start) - (note in "Additional Submission Details" field)
- REQUIRED no exemptions - Document care using the Epic EMR system
- ACLS
- BLS
- Board Certified
- Board Eligible
- DEA
- Colorado
- DEA Controlled Substance License (required once hired), BLS/ACLS Certification (standard requirement for hospital practice)
