Gentiva is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to join our team in Colorado. You will provide hands-on nursing care to patients and support families in homes, assisted living, or facility settings, reporting to the Administrator or Executive Director.
Responsibilities include direct nursing care, care plan execution, timely reporting to the RN, and collaboration with the interdisciplinary team to ensure comfort and quality of life for patients at the end of life.
