Gentiva is seeking a Hospice RN / RN Case Manager to lead patient care in home, assisted living, or similar settings in the Pueblo area. You will assess patients, administer skilled nursing care, and coordinate with an interdisciplinary team to optimize comfort and quality of life for patients and their families.
Qualified candidates have at least 1 year as an RN with hospice or home health experience preferred, a current RN license for the state, and CPR certification.
