Overview

The Resource Exchange (TRE) is seeking a dedicated Service Coordinator to assist and synchronize supports and services for individuals within a dynamic, highly regulated environment. The role involves building meaningful relationships with clients, families and community partners while managing service planning, monitoring activities and ensuring compliance with all regulations. The Service Coordinator must remain agile, responsive and aligned with TRE’s Mission, Vision and Values.

Key Responsibilities
  • Promote excellent internal and external customer service, reflecting TRE’s values through intentional communication.
  • Develop a strong understanding of the rules and regulations that direct Long Term Care (LTC) services and maintain compliance.
  • Organize and implement service‑plan development tasks in person, by phone or via technology (Outlook, Office 365, ZOOM, etc.).
  • Monitor authorized services and utilization to promote person‑centered practices while ensuring the health, safety and personal autonomy of the people served.
  • Cultivate an understanding of local and extended community resources to support additional client needs.
  • Attend departmental and agency meetings and trainings in person, by phone or through technology as specified.
  • Perform service‑plan development tasks and duties in and around the community, using company‑provided or personal transportation.
  • Maintain driving responsibilities, including a valid Colorado driver’s license and minimum insurance coverage.
  • Provide coverage for team members as needed to maintain quality and continuity of services.
  • Participate in professional goal creation and pursue annual career development plans.
  • Adapt to changing work environments due to new state and federal rules and regulations.
  • Assume additional duties as required to promote the health and wellbeing of clients and the organization.
Required Knowledge, Skills & Abilities
  • Proficient with computer‑based technology (Apple iPhone, laptops, tablets, Microsoft 365, Outlook, Skype for Business, Adobe Acrobat, etc.).
  • Strong understanding of case‑management best practices and person‑centered thinking.
  • Accurate typing and detail orientation while processing data.
  • Positive attitude, friendly disposition and assuredness in professional interactions.
  • Empathy for people with long‑term care support needs.
  • Problem‑solving and critical thinking skills.
  • Strong organizational skills with the ability to prioritize multiple daily tasks and emergencies.
  • Cultural awareness and understanding of workplace biases.
Education & Experience Requirements
  • Minimum:
    Bachelor’s degree in human behavioral science or a related field.
  • Preferred:
    Working knowledge of Medicaid Waiver programs, experience with persons with disabilities, and familiarity with community resources.

Working Environment
and Physical Activities

Office, community, private residences, commercial properties, and virtual environment settings. Moderate physical activity required (walking, sitting, standing, lifting up to 20 pounds).
