Job Summary and Responsibilities- Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
- Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
- Provide purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
- Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.
- Valid physical therapy license in the state(s) where services are rendered.
- One (1) year healthcare experience in any capacity (preferred).
- BLS from an approved agency within 3 months of hire/transfer.
- Graduate of an accredited program of physical therapy, culminating in a minimum of a bachelor‑of‑science degree.
- Effective verbal and written English communication skills.
- Demonstrated basic to intermediate skills in Microsoft Word, Internet and Intranet navigation.
- Highest level of professionalism with the ability to maintain confidentiality.
- Good communication skills including ability to handle complaints, objections and resolve conflicts.
- Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
- Customer service oriented with the ability to work well under pressure and demonstrate the ability to field intermittent interruptions while providing optimal care to patient.
- Deliver service through a team approach that includes all disciplines, team members and patients.
