• Help patients maintain, restore, and improve movement, activity, and function.
• Provide interventions, conduct reexaminations (every 30 days, at minimum), modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.

• Current license to practice physical therapy in the state of assignment.
• Graduate from an accredited physical therapy program (CAPTE).
• One year related PT experience for home health care.
• If less than one year, regional clinical leader approval and appropriate mentoring or residency participation required.
• Current CPR certification.
