Responsibilities- Assess and screen patients' daily living and work‑related skills, develop therapeutic retraining programs with measurable objectives, and administer and interpret diagnostic and prognostic tests in patients' homes.
- Confer with the patient's physician and other health‑care team members, participating in development and revision of the Plan of Care.
- Provide therapeutic treatment and instruction to patients according to physician orders to improve or restore strength, coordination, range of motion, and function, or teach compensation measures.
- Review and expand retraining programs consistent with each patient's behavioral goals and overall rehabilitation program guidelines.
- Consult with vocational team members to develop and implement vocationally oriented plans matching patients' needs and capabilities.
- Report patient status and progress to physician, rehabilitation staff, Clinical Manager, patients, and family members.
- Participate in care coordination and discharge planning activities, acting as a resource to other health‑care personnel in meeting patients' needs.
- Design community reintegration activities to aid physical reconditioning and psychological adjustment, coordinating plans with interdisciplinary team members.
- Recommend or design special adaptive equipment to improve clients' residential or working environments and enhance participation in the rehabilitation program.
- Design and train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
- Coordinate, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in delivering occupational therapy services.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations on the day services are rendered; ensure interim (verbal) orders from the physician are implemented and documented; submit evaluations, treatment plans, and discharge summaries to supervisor and care‑management staff.
- Degree from an accredited Occupational Therapy Program.
- Minimum of six months of occupational therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
