Gentiva Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to provide hands-on hospice care in patients’ homes, assisted living, or facility settings. You will support the interdisciplinary team, assess needs, and deliver care per the plan while reporting changes to the RN and care team.
Ideal candidates have an accredited LPN program background, an active LPN license in the state, CPR, and 3–5 years of hospice/home health experience.
